Advancing cross-sector collaboration on the virtual twin for CAR T cell therapy, CERTAINTY enters its third year with ongoing implementation across work packages
As of December 2025, the Horizon Europe project CERTAINTY has officially entered its third year. Launched in December 2023, CERTAINTY brings together 17 partners across Europe and beyond, working to develop a patient-specific virtual twin to support decision-making in CAR T cell therapy.
The project focuses on multiple myeloma as its primary clinical use case, combining biomedical research, AI-driven modelling, data ethics, and patient engagement into a single, integrated initiative. Led by Fraunhofer IZI, the CERTAINTY consortium includes universities, hospitals, SMEs, a patient organisation, and research centres united by a shared ambition: to improve the precision and impact of immunotherapy through virtual twin technology.
A Steady Transition from Planning to Implementation
During its first two years, CERTAINTY focused on establishing its foundations: technical architecture, initial data governance and ethics planning, and stakeholder engagement strategies. As the project enters Year 3, work packages are now deeply engaged in implementation activities. Scientific and clinical partners are progressing on modelling approaches, while technological partners refine software infrastructure. Meanwhile, stakeholder and ethical frameworks continue to be shaped through active dialogue and contribution from patients, legal experts, and healthcare professionals.

Several important events are scheduled in the coming months:
CERTAINTY Hackathon – February 2026
This internal hackathon will bring together partners to address two key priorities: preparing the common data model for federated learning and developing a first prototype of the user interface for CAR T treatment eligibility. The event will foster collaboration across work packages, accelerate technical alignment, and support the design of a usable and explainable virtual twin interface.
Annual Consortium Meeting – March 2026
CERTAINTY’s annual meeting will take place in March, bringing together all project partners to review progress across work packages, align on current challenges, and engage in open discussions. The agenda will include roundtables, strategic planning, and agreement on next steps as the project continues its implementation phase.
Looking Ahead
While much work remains, the CERTAINTY consortium remains committed to its core principles: scientific excellence, responsible innovation, and stakeholder co-creation. Entering Year 3, the project continues to build on the strength of its interdisciplinary collaboration, ensuring each partner’s expertise contributes to the virtual twin’s future applicability in real-world settings.
